Please note that application requirements vary depending on the cumulative number of reissues since the date of CIS account registration.

| 3rd time | A full-time faculty member of this center must approve the reissue application (sign the form). |
| 4th time | |
| 5th time or more | The reissue application must be approved by the University’s Information Security Operations Office. |
As a general rule, reissued passwords will be issued in person from the business day following the day the application is received.
(Passwords will be reissued on the same day at the request of the person in charge of the seminar only in unavoidable circumstances, such as when the password is for a seminar.)
